Quality of life for elderly family members can unexpectedly change due to health issues, financial challenges or some loss of independence, leaving adult children or elderly spouses having to pick up the pieces of unprepared arrangements for the care of their parents or partner.

Experiencing little in the way of coordinated, holistic caregiver support in the broader community, Aging With Grace has developed customizable programs to assist in the preparation, planning, and implementation of the four primary areas of support: Legal, Financial, Home, and Personal.

Book a Family Caregiver Assessment

A Family Caregiver Assessment is an in-depth look at the current situation you and your elderly loved ones are facing. First identifying the most immediate needs to address helps to establish a starting point. This provides the foundation on which to create planning around longer term goals for both you and your family member in need of care.

The assessment will clarify a number of key areas including:

​

  • Understanding what you the caregiver are investing in your time and energy

  • Identifying the current familycare dynamics and desired outcomes

  • Establishing if critical legal documents are in place, ie: Power of Attorney 

  • Confirming if standing fiancial documents allow caregiver access, ie: Wills, Estates & Trusts, or bank accounts 

  • Documented medical forms for emergency care such as Assisted Living and Insurance claims

  • Desired quailty of life choices and stages, ie: Independant Living or In-home Support

Once complete, a Caregiver Action Plan will be created to show how we can best provide support and mentoring for caregivers.

Why Plan Ahead?

Creating a living family plan while parents and adult children are able to address important issues before something happens is ideal. Understanding desired wishes and the necessary legal and medical issues such as having a power of attorney, wills, and estates, and health plans in place create a structure that eliminates guessing what to do when the time comes to support the needs of each family member. Caregiver Action Plans help everyone clearly know what must take place when the time comes.
